The Tojak 5mg tablet contains Tofacitinib citrate, which is an immunosuppressant and Janus kinase (JAK) inhibitor used to treat specific autoimmune and inflammatory conditions. It is manufactured by the pharmaceutical company Cipla.

Key Information

  • Active Ingredient: Tofacitinib 5mg (as tofacitinib citrate INN).
  • Therapeutic Class: Immunosuppressant, specifically a Janus kinase (JAK) inhibitor.
  • Mechanism of Action: It works by blocking the action of the Janus kinase enzyme, which in turn decreases the activity of the immune system and the inflammation associated with various rheumatic diseases and ulcerative colitis.

Primary Uses

Tojak 5mg is a prescription medication used when previous treatments (like methotrexate or TNF blockers) have not worked well or were not tolerated:

  • Rheumatoid Arthritis: For adults with moderately to severely active rheumatoid arthritis.
  • Psoriatic Arthritis: For adults with active psoriatic arthritis, often used in combination with nonbiologic disease-modifying antirheumatic drugs (DMARDs).
  • Ulcerative Colitis: For adults with moderately to severely active ulcerative colitis.
  • Ankylosing Spondylitis: To treat active ankylosing spondylitis in adults.
  • Polyarticular Course Juvenile Idiopathic Arthritis (pcJIA): The medication (in various forms) is also approved for patients 2 years of age and older with active pcJIA.

Important Safety Information and Warnings

Tofacitinib affects the immune system, which can increase the risk of serious side effects. It is important to discuss your medical history with your doctor before starting treatment.

  • Serious Infections: Tofajak can make you more susceptible to infections, including tuberculosis (TB), bacterial, fungal, and viral infections. Your doctor will screen you for TB before beginning treatment.
  • Blood Clots: There is an increased risk of blood clots in the lungs (pulmonary embolism) and legs (deep vein thrombosis), especially in individuals over 50 years old with heart disease risk factors.
  • Cancer Risk: The medication may increase the risk of certain cancers, including lymphoma and skin cancers.
  • Gastrointestinal Perforations: Use with caution in patients at increased risk of tears (perforations) in the stomach or intestines.
  • Laboratory Monitoring: Regular blood tests are necessary to monitor blood counts, liver function, and cholesterol levels while taking this medication.
